Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Position Summary:
The Licensed Practical Nurse (LPN) provides high-quality nursing care to residents in a long-term care setting. This role includes monitoring patient conditions, administering medications, coordinating with interdisciplinary teams, and supporting daily clinical operations to ensure the safety, comfort, and well-being of residents.
Key Responsibilities:

Provide direct nursing care to residents in accordance with facility policies and state guidelines.

Administer medications and treatments as ordered by the physician.

Monitor vital signs, observe resident conditions, and report changes promptly to the RN or provider.

Maintain accurate and timely documentation in EMR/charting systems.

Assist with admissions, transfers, discharges, and care plan updates.

Communicate effectively with residents, families, and interdisciplinary team members.

Support CNAs and ensure residents receive appropriate personal care.

Participate in daily shift reports and handoffs to maintain continuity of care.

Respond to medical emergencies and follow appropriate protocols.

Maintain infection control, safety standards, and compliance with regulatory requirements.

Provide emotional support and compassionate care to residents and their families.
Qualifications:

Valid and active LPN license (state-specific or compact).

Current BLS/CPR certification.

Previous LTC, SNF, or geriatric nursing experience preferred but not always required.

Strong communication, assessment, and time-management skills.

Ability to work in a fast-paced environment and manage multiple residents.
Pay Rate
$32.31 - $37.19/hour
