Social Worker
We are seeking a compassionate and dedicated Social Worker to support patients and families by assessing psychosocial needs, coordinating resources, and assisting with care planning. This role involves providing emotional support, crisis intervention, and advocacy while collaborating with healthcare teams to ensure safe and effective patient outcomes.
Responsibilities:

Assess patient and family needs, identify barriers, and develop appropriate support plans.

Provide counseling, emotional support, and crisis intervention as needed.

Coordinate community resources, discharge planning, and referrals to appropriate services.

Collaborate with healthcare teams to support patient care goals and ensure continuity of care.

Document assessments, interventions, and service plans accurately and timely.

Advocate for patients and families to ensure access to necessary programs and services.

Support patients in coping with illness, hospitalization, and social challenges.
Qualifications:

Bachelor’s or Master’s degree in Social Work (depending on role requirements).

Active and unrestricted state licensure, if required for the position.

Prior experience in healthcare, behavioral health, or community services preferred.

Strong communication, assessment, and problem-solving skills.

Ability to work effectively with multidisciplinary teams and diverse populations.
Pay Rate
$41.28 - $46.16/hour
