Med/Surg and Progressive Care; ACLS is required and at least 5 years of experience.
JOB SUMMARY
Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population.
Performs patient assessment/reassessment of health conditions through interviews, observation and inspection in a systematic, organized manner. Indiana Nurse Practice Act (INPA)
Formulates a nursing diagnosis based on accessible, communicable, and recorded data which is collected in a systematic and continuous manner. (INPA)
Develops and coordinates an individual plan of care including discharge planning, goals and prioritized nursing approaches or measures derived from the nursing diagnosis. (INPA)
Communicates, collaborates and works with other members of the health team to provide safe, effective, quality and continuity of care. (INPA)
Implements strategies to provide for patient participation in the plan of care, health promotion, maintenance and restoration. (INPA)
Initiates nursing actions to assist the patient to maximize his or her health capabilities. (INPA)
Recognizes changes in patient conditions and responds appropriately. Informs the physician of changes in the patient’s condition and modifies the plan as needed. (INPA)
Evaluates with the patient the status of goal achievement as a basis for reassessment, reordering priorities, new goal-setting, and revision of the plan of nursing care. (INPA)
Delegates appropriately to subordinates in accordance with the legal scope of practice, job description, and competency level of the subordinates. Effectively supervises nursing care being given by subordinates. Retains professional accountability for nursing care when delegating nursing interventions. (INPA)
Complete documentation of patient care according to PMH standard procedure.
Develops, implements, coordinates, and documents education for patient/family/significant other.
Administers medication according to PMH policy and procedure following the 5 Rights of Safe Medication Practice; has knowledge of drug indications, contraindications and side effects. (HFAP/CMS)
Administers and maintains intravenous therapy according to PMH policy and procedures. (HFAP/CMS)
Administers blood and blood products according to PMH policy and procedure. (HFAP/CMS)
Applies restraints in accordance with physician orders and PMH policy and procedure. Uses de-escalation techniques appropriately when needed. (HFAP/CMS)
Performs treatments, therapies and operates equipment according to policy and procedure. Assists physician with treatment and procedures and special tests as required and according to policy. Seeks education and supervision as necessary when implementing nursing practice techniques. (INPA)
Assists in the education and development of staff.
Participates in performance improvement initiatives and demonstrates the use of quality improvement in daily operations. (INPA)
Complies with established PMH Board, Hospital and Departmental policies and procedures.
Seeks educational resources and create learning experiences to enhance and maintain current knowledge and skill for his or her continuing competence in nursing practice and individual professional growth. (INPA)
Assumes responsibility for individual nursing actions and continued professional growth and attends variety of meetings, conferences, and seminars as required or directed. (INPA)
Notifies the manager of any unprofessional conduct or unsafe situations which may jeopardize patient safety. (INPA)
Respects the dignity and rights of patient/client regardless of socioeconomic status, personal attributes, or nature of health problems. Provides nursing care without discrimination on the basis of diagnosis, race, color, age, sex, national origin, religion, gender identity, sexual orientation, ethnicity, linguistics, source of payment, or disability, (INPA)
Maintains each patient/client right to privacy by protecting confidential information and adheres to hospital policy on confidentiality and privacy of protected health information.
Performs other similar and related duties as required or directed
MINIMUM KNOWLEDGE AND SKILL REQUIREMENTS
Completion of competencies required in the area(s) in which they work.
Knowledge of nursing care principles, procedures, and practices.
Knowledge of pharmaceutical, expected beneficial effects, potential side effects and be skilled safe administration practices.
Ability to recognize adverse patient signs and symptoms and to react appropriately.
Must be able to identify and provide for age-specific patient needs.
Must be able to assess, interpret, and appropriately react to key data about a patient’s status.
Skill in operating job related medical equipment.
DEPARTMENT SPECIFIC REQUIREMENTS:

EXPERIENCE AND EDUCATION:
MINIMUM REQUIRED:
Graduated from an Accredited School of Nursing. Bachelor’s of Science in Nursing is preferred, but associate and diploma is accepted. Minimum 5 years of experience.
LICENSES/CERTIFICATIONS:
REQUIRED: Holds a current Indiana Registered Nurse license and current BLS – Healthcare provider’s certification.
