Position Summary:
We are seeking an experienced RN Case Manager to join a dynamic healthcare team focused on improving patient outcomes through innovative and efficient care management. The RN Case Manager will coordinate care, manage chronic disease workflows, and collaborate with providers to enhance clinical and operational performance.
Key Responsibilities:
• Manage high-risk and chronic disease patients using evidence-based protocols.
• Coordinate care transitions, follow-up appointments, and wellness initiatives.
• Implement and monitor patient engagement and remote care programs.
• Audit clinical workflows to identify opportunities for quality and efficiency improvements.
• Collaborate with multidisciplinary teams to ensure compliance with care standards.
• Maintain accurate records and participate in performance reporting and process improvement.
Required Qualifications:
• Active Registered Nurse (RN) license (compact license preferred).
• Minimum 3 years of clinical experience in internal medicine, family medicine, or geriatrics.
• Prior experience in case management, population health, or care coordination.
• Proficiency with EHR systems and virtual care tools.
• Strong communication, analytical, and leadership skills.

Pay Rate
$28.60 - $31.80/hour
