Roles:
- Provide in-home nursing care to adult patients with chronic illnesses, disabilities, or post-surgical needs.
- Conduct patient assessments and develop individualized care plans.
- Administer medications, perform wound care, and monitor vital signs.
- Educate patients and caregivers on disease management and treatment plans.
- Maintain accurate documentation and coordinate care with healthcare providers.
Two years of previous acute care experience in MedSurg, Maternal and child care, Critical Care, Pediatrics.
Current Adult Care home health experience
