Registered Nurse (RN) – Case Manager
We are seeking an experienced and compassionate Registered Nurse Case Manager to coordinate and oversee patient care across the continuum of services. The RN Case Manager will assess patient needs, develop and manage individualized care plans, coordinate services, monitor progress, and collaborate with interdisciplinary teams to ensure safe, efficient, and high-quality patient-centered outcomes.
Responsibilities:
• Conduct comprehensive patient assessments and identify clinical and psychosocial needs
• Develop, implement, and manage individualized care plans
• Coordinate care across healthcare providers, services, and settings
• Monitor patient progress, outcomes, and adherence to care plans
• Advocate for patients to ensure appropriate resources and services are provided
• Educate patients and families on care plans, treatments, and available resources
• Communicate effectively with physicians, nurses, social workers, and other team members
• Maintain accurate, timely, and compliant documentation
Qualifications:
• Active and unrestricted Registered Nurse (RN) license
• Nursing degree (Associate’s or Bachelor’s preferred)
• Experience in case management, care coordination, or clinical nursing preferred
• Strong assessment, care planning, and organizational skills
• Excellent communication, critical thinking, and patient advocacy abilities
• Ability to manage multiple cases and priorities effectively
• Commitment to high-quality, patient-centered care
Pay Rate
$67.58 - $72.46/hour
