**Candidate must be open to going perm after the initial contract period**

The RNCM establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination and monitoring, and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities includes: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from acute care centers and skilled nursing facilities. Additionally, this position will work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure cohesive decisions are made which ensure the safety and well-being of participants.

**This position is primarily on site, but there are times when RNCM will need to drive to a participants residence for an assessment if needed. Mileage will be reimbursed for this time.** RNCM will start from the center and leave to the community to do those visits.

Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more.

Locals and True Travel ok!! Prefer 26 week contract

All the RNCM’s are assigned a laptop and cell phone to perform their duties. We use a system called Concur where the RNCM can create an expense report and track miles.
