Position Summary:
An RN Case Manager develops, implements, and reviews healthcare plans for patients that are geriatric, recovering from serious injuries, or dealing with chronic illnesses. Case managers work both within and outside of a hospital or medical facility. And these registered nurses (RNs) collaborate with doctors and other medical professionals to give their patients the comprehensive care that they need. This includes advocating for their patients, coordinating their care, and providing other healthcare services and education.
Floating Expectation: Float to the Laguna Facility
Ratios: 1:25
Weekend REQ: Weekend Rotation
Key Responsibilities:
- Conducting intake assessments with patients and families.
- Ensuring that families feel sufficiently equipped and supported.
- Developing individualized treatment plans that account for physical, financial, and other constraints.
- Adjusting treatment protocols, as needed.
- Suggesting and coordinating appropriate follow-up services to promote wellness.
- Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities.
- CA State RN license in hand
- Basic Life Support (BLS) certification.
- 2-year acute medical Care Manager experience performing Care Coordination
- Travel experience required
Pay Rate
$70.70 - $73.90/hour
