- SNFs covered: Bethany and Woodland in Brainerd; Pine River SNF; Nisswa HH
- Is Home Health involved? Occasionally- up to 5/day; Radius of coverage- 40 miles
- Productivity expectations- 80%
- Will the PT supervise others? Yes, up to 2 PTAs
- Mileage reimbursement begins after 1st work location
Bill Rate will revert to regular rate for extension.
