PT Nursing and Rehabilitation Nurse
We are seeking a compassionate and patient-focused PT Nursing and Rehabilitation Nurse to provide skilled nursing care and support to patients undergoing physical therapy and rehabilitation services. The nurse will assist with clinical care, monitor patient progress, reinforce therapy goals, and collaborate with the interdisciplinary rehabilitation team to promote recovery, functional independence, and optimal patient outcomes.
Responsibilities:
Provide direct nursing care to patients participating in physical therapy and rehabilitation programs.
Assess patient conditions, monitor vital signs, pain levels, mobility status, and response to therapy.
Support and reinforce physical therapy and rehabilitation treatment plans as directed.
Administer medications, treatments, and basic nursing interventions as prescribed.
Assist patients with mobility, transfers, exercises, and activities of daily living as appropriate.
Educate patients and families on rehabilitation goals, safety precautions, and care plans.
Document nursing assessments, interventions, and patient progress accurately and timely.
Collaborate with physical therapists, occupational therapists, physicians, and care team members to ensure coordinated care.
Maintain a safe, supportive, and encouraging environment for patient recovery.
Qualifications:
Active and unrestricted nursing license (RN or LPN, as applicable).
Experience in rehabilitation, physical therapy support, post-acute care, or skilled nursing preferred.
Knowledge of rehabilitation principles, mobility assistance, and patient safety practices.
Strong assessment, communication, and teamwork skills.
Ability to work effectively with patients recovering from injury, surgery, or illness.
Compassionate, dependable, and committed to patient-centered rehabilitation care.
Pay Rate
$54.37 - $59.25/hour
