Patient Enrollment Specialist

Job Summary:

We are seeking a compassionate and professional Patient Enrollment Specialist to join our healthcare technology team. This role blends patient interaction with administrative efficiency, offering hands-on experience in chronic care support programs. The ideal candidate will serve as the face of the organization at a partner medical office and also provide support at the corporate headquarters.

Key Responsibilities:

• Act as the primary representative for chronic care programs on-site at a local medical practice.
• Welcome and engage patients with warmth, explaining program benefits clearly and empathetically.
• Conduct phone outreach and follow-up with patients for enrollment.
• Secure patient consents for chronic care programs in compliance with regulations.
• Educate patients on remote care devices and participation expectations.
• Maintain accurate enrollment records via CRM systems.
• Coordinate communication through email, SMS, and internal platforms.
• Collaborate closely with medical staff and the internal clinical team to ensure smooth onboarding.
• Meet daily/weekly performance metrics including enrollments and patient satisfaction.
• Report to HQ periodically for training, support, and collaboration.

Qualifications:

Required:

• High school diploma or equivalent
• Minimum 1 year experience in customer service, call center, or patient-facing roles
• Ability to work on-site daily (Laguna Hills, with some days in Newport Beach)
• Typing speed of 40 WPM
• Proficiency with communication tools and CRM software
• Strong written and verbal communication skills
• Detail-oriented, organized, and professional in demeanor

Preferred:

• Bilingual in Spanish, Farsi, Korean, or Mandarin
• Experience in healthcare settings or CRM platforms such as HubSpot
• Familiarity with performance-based goals and metrics
