Outpatient Physical Therapist
We are seeking a skilled and patient-focused Outpatient Physical Therapist to provide high-quality rehabilitation services in an outpatient care setting. The therapist will evaluate functional limitations, create individualized treatment plans, and deliver therapeutic interventions to support recovery, mobility, and long-term functional wellness.
Responsibilities:

Perform comprehensive evaluations to assess strength, mobility, balance, and functional limitations.

Develop and implement individualized outpatient treatment plans tailored to patient goals.

Provide therapeutic exercises, manual therapy, gait training, and functional mobility interventions.

Educate patients and families on home exercise programs, injury prevention, and recovery expectations.

Track patient progress, update plans of care, and maintain accurate clinical documentation.

Collaborate with referring providers and care team members to ensure coordinated, patient-centered care.

Promote a safe, supportive rehabilitation environment and encourage patient engagement.
Qualifications:

Active and unrestricted Physical Therapist (PT) license.

Experience in outpatient rehabilitation strongly preferred.

Strong clinical skills in evaluation, therapeutic exercise, and patient education.

Excellent communication, organization, and critical-thinking abilities.

Pay Rate
$64.28 - $69.16/hour
