Position Summary:
We are seeking a full-time Primary Care Nurse Practitioner (NP) to deliver high-quality medical services to patients residing in long-term care and skilled nursing facilities. This role is designed for a compassionate clinician with experience managing chronic conditions and acute changes in elderly populations. You will work collaboratively with an interdisciplinary care team, ensuring continuity of care and quality outcomes.
Key Responsibilities:
• Provide primary and preventative care to long-term care residents.
• Assess and manage acute and chronic medical conditions.
• Conduct advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances per OH law.
• Order, review, and follow up on diagnostic testing and treatments.
• Collaborate with facility staff and physicians to optimize patient care.
• Maintain accurate EMR documentation and participate in care team communication.
Required Qualifications:
• Current, unrestricted Nurse Practitioner license in Ohio.
• Board Certification (Adult, Family, or Geriatrics).
• DEA licensure (required to prescribe controlled substances).
• Bachelor's degree in Nursing (RN to BSN).
• Preferred: Master's degree in Adult-Gerontology NP specialization.
• Clinical experience in adult, family, or geriatric care.
• Proficiency in using EMR systems.
• Experience managing patients with advanced illnesses and multiple chronic conditions.
• Psych experience is a plus.

• Day shift (8:00 AM – 5:00 PM)
• 60-minute unpaid lunch break
