About the Opportunity
We are seeking a compassionate and experienced Nurse Practitioner to provide primary and preventive care for adult patients residing in long-term care settings. You will be part of an interdisciplinary care team focused on improving the quality of life and health outcomes for patients in skilled nursing environments.

Key Responsibilities
• Deliver primary and preventive care services to patients in skilled nursing settings
• Manage acute changes in patient conditions
• Conduct chronic care management and advanced care planning
• Prescribe medications, including controlled substances in compliance with state law
• Order and interpret diagnostic tests and treatments
• Collaborate with nursing staff and other healthcare professionals to adjust care plans as needed
• Communicate clinical updates and recommendations clearly and regularly
• Coordinate with primary care physicians to ensure continuity of care

Required Qualifications
• Active and unrestricted NP license in North Carolina
• Board Certification (NP)
• DEA registration for prescribing controlled substances
• Experience in long-term care, adult, family, or geriatric medicine
• Proficiency with Electronic Medical Records (EMR)
• Ability to manage patients with advanced illness and chronic conditions
• Strong communication and teamwork skills to function in an interdisciplinary environment

Education Requirements
• Master’s Degree in Adult-Gerontology Nurse Practitioner specialization
• Bachelor’s Degree in Nursing (RN to BSN)
• High School Diploma

Preferred Skills
• Familiarity with long-term care protocols
• Background in psychiatric care (preferred but not required)
