Job Overview
We are seeking an experienced Nurse Manager – Case Management to lead and support a multidisciplinary team across an acute care setting. This leadership role requires strong experience overseeing Case Managers and Social Workers, with responsibility for workflow coordination, staff supervision, patient throughput, and maintaining efficient discharge planning processes.
The ideal candidate is a strategic, collaborative, and hands-on leader who thrives in a fast-paced hospital environment.
Key Responsibilities
- Provide daily leadership, support, and oversight for the Case Management and Social Work teams.
- Manage staffing, scheduling, workflow prioritization, and operational efficiency within the department.
- Oversee discharge planning, care transitions, and patient continuity of care.
- Collaborate with physicians, nursing leadership, and interdisciplinary teams to ensure timely patient movement and high-quality care.
- Ensure compliance with hospital policies, regulatory requirements, and documentation standards.
- Conduct performance evaluations, mentor staff, and implement improvement initiatives as needed.
- Utilize MEDITECH (version 6.1) for clinical documentation and workflow management.
Required Experience & Skills
- Minimum 4 years of relevant management experience.
- Prior Director-level experience in Case Management is required.
- Strong leadership and communication skills with the ability to guide diverse clinical teams.
- Demonstrated experience with complex discharge planning and care coordination.
- Proficiency in MEDITECH or ability to quickly adapt to electronic medical records systems.
Certifications & Licensure
- Active RN License (state-specific requirement).
- BLS Certification – Required.
- BSN Not Required.
Pay Rate
$94.48 - $97.68/hour
