Physical Therapist – Home Health (ND)
We are seeking a dedicated and compassionate Physical Therapist to provide high-quality rehabilitative care to patients in their homes. The PT will assess mobility and functional needs, develop personalized treatment plans, deliver therapeutic interventions, and support patients in achieving improved independence and quality of life while working independently in a home health setting.
Responsibilities:

Perform comprehensive physical therapy evaluations and develop individualized care plans.

Provide therapeutic exercises, gait training, balance training, and mobility interventions in the patient’s home.

Monitor patient progress, adjust treatment plans, and document all care accurately.

Educate patients and caregivers on home exercise programs, safety techniques, and mobility strategies.

Coordinate care with nurses, occupational therapists, speech therapists, and other team members.

Communicate changes in patient condition and therapy needs to the care team as appropriate.
Qualifications:

Current state license as a Physical Therapist (active and unrestricted).

Experience in home health or community-based therapy preferred.

Strong clinical assessment and care-planning skills.

Excellent communication, documentation, and time-management abilities.

Ability to work independently and provide patient-centered care in a home environment.
Pay Rate
$66.57 - $71.44/hour
