Occupational Therapist – Home Health (ND)
We are seeking a dedicated and compassionate Occupational Therapist to provide high-quality rehabilitative care to patients in a home health setting. The OT will evaluate functional abilities, develop personalized treatment plans, and deliver therapeutic interventions that help patients achieve greater independence and improve their daily living skills in the comfort of their homes.
Responsibilities:

Conduct comprehensive home-based evaluations to identify patient needs and functional limitations.

Develop and implement individualized treatment plans to enhance mobility, safety, and independence.

Provide therapeutic activities, training in self-care tasks, and instruction in adaptive equipment use.

Monitor progress, adjust care plans as needed, and maintain accurate documentation.

Educate patients and caregivers on home exercise programs, safety strategies, and condition management.

Collaborate with nurses, physical therapists, speech therapists, and other care team members to ensure coordinated home-based care.
Qualifications:

Current Occupational Therapist license (active and unrestricted) in North Dakota.

Experience in home health or rehabilitation settings preferred.

Strong skills in assessment, treatment planning, and patient education.

Excellent communication, independence, and time-management skills.

Compassionate, patient-focused, and able to deliver therapeutic care effectively in a home environment.
Pay Rate
$62.75 - $67.63/hour
