LPN – Long-Term Care (LTC) Job Description
Job Title: Licensed Practical Nurse (LPN)
Department: Long-Term Care
Reports To: Registered Nurse (RN) / Nursing Supervisor
Job Summary:
The LPN in a Long-Term Care facility provides direct nursing care to residents under the supervision of an RN or physician. Responsibilities include monitoring health conditions, administering medications, assisting with daily living activities, and ensuring a safe and supportive environment for all residents.
Key Responsibilities:

Provide direct patient care to residents in accordance with the care plan.

Administer medications, treatments, and injections as prescribed.

Monitor and document residents’ vital signs, symptoms, and changes in condition.

Assist residents with daily living activities (bathing, dressing, mobility, hygiene).

Maintain accurate and timely medical records and documentation.

Follow infection control and safety protocols.

Assist RNs and physicians during assessments, procedures, and care planning.

Communicate effectively with residents, families, and the healthcare team.

Report any changes in residents’ conditions to the RN or supervisor promptly.

Ensure a clean, safe, and comfortable environment for residents.
Qualifications:

Active and valid LPN license.

CPR/BLS certification.
-
Experience in long-term care or geriatrics preferred but not required.

Strong communication, organizational, and teamwork skills.
Pay Rate
$29.33 - $32.53/hour
