Job Description: LPN – Long-Term Care (LTC)

The Licensed Practical Nurse (LPN) in a Long-Term Care (LTC) setting is responsible for delivering high-quality nursing care to residents while ensuring their safety, comfort, and well-being. This role involves coordinating with healthcare teams, administering treatments, and supporting daily living needs in accordance with facility standards and state regulations.

Key Responsibilities

Provide direct nursing care to residents, including administering medications, monitoring vital signs, and performing treatments.
Assist residents with activities of daily living (ADLs) and ensure their comfort and safety.
Accurately document care provided, resident status, and changes in condition using facility charting systems.
Collaborate with RNs, physicians, and interdisciplinary teams to update and implement individualized care plans.
Respond promptly to resident needs, emergencies, and changes in health status.
Maintain compliance with all infection control protocols, facility policies, and state regulations.
Provide compassionate resident interaction and support to families regarding care updates.
Participate in shift handoffs, team meetings, and ongoing training to ensure continuity and quality of care.

Qualifications

Active LPN/LVN license in the applicable state (or compact).
Previous experience in LTC, SNF, or geriatric care preferred.
Strong assessment, communication, and time-management skills.
Ability to work independently and as part of a multidisciplinary team.
Knowledge of infection control, medication administration, and electronic charting.

Pay Rate
$28.60 - $31.80/hour
