Job Title: Licensed Practical Nurse (LPN)
Job Summary:
The Licensed Practical Nurse (LPN) provides basic nursing care under the supervision of a Registered Nurse (RN) or a Physician. The LPN ensures high-quality patient care, supports daily medical needs, and assists in maintaining accurate health records while promoting a safe and comfortable environment for patients.
Responsibilities:

Provide direct patient care according to the plan of care established by the RN or physician.

Administer medications and treatments as prescribed.

Monitor and record patient vital signs, conditions, and responses to treatment.

Assist with admissions, discharges, and transfers of residents/patients.

Maintain accurate and timely documentation in patient medical records.

Communicate effectively with patients, families, and healthcare team members.

Report any changes in patient condition to the supervising RN or physician.

Ensure infection control and safety protocols are followed at all times.

Support patients with daily living activities (ADLs) when needed.
Qualifications:

Valid and active LPN/LVN license in the state of employment.

BLS/CPR certification (required).

Prior experience in LTC, Rehab, or Skilled Nursing Facility preferred.

Strong communication, organizational, and assessment skills.

Compassionate and dependable with attention to patient needs.
Pay Rate
$27.32 - $30.52/hour
