Position Summary: We are hiring a Family Nurse Practitioner (FNP) to provide full-scope primary and mental health care in Portland, OR. The FNP will assess, diagnose, treat, and educate patients across the lifespan. This role includes independence, collaboration with care teams, and focus on wellness and chronic care management.
Key Responsibilities:
• Perform physical and mental health assessments
• Diagnose and manage acute/chronic conditions
• Order tests and prescribe medications
• Provide patient education and wellness counseling
• Support mental health through assessments and medication management
• Collaborate with interdisciplinary teams and manage patient cases
Requirements:
• Master’s or Doctorate in Nursing (FNP focus)
• RN license in Oregon & national FNP certification
• 2 years' experience in assessment, treatment planning, and medication management
• Excellent communication, empathy, and teamwork skills
Schedule:
40 hours/week, Mon–Fri or Tue–Sat (8AM–5PM or 9AM–6PM)
