What You’ll Do
Evaluate and treat residents in skilled nursing and post-acute settings.
Develop and implement individualized treatment plans to help residents achieve their highest functional potential.
Collaborate with an interdisciplinary team to ensure seamless, coordinated care.
Document accurately and timely in accordance with facility and regulatory standards.
Participate in care conferences and contribute to ongoing quality improvement efforts.
What We’re Looking For
Active California Physical Therapist license (required)
Strong clinical and communication skills
Skilled nursing or post-acute experience preferred
Dependable, flexible, and team-oriented
