Job Description:
Primary Care Clinic at Tsehootsooi Medical Center.
Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:
Requirement: Three (3) year Case Management Experience.
Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.
Housing: Information can be found under the 'Facility Group Document’ section.
Orientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.
